YOUR LOCATION
This helps me understand how we might work together. Lewis Legacy Law Studio primarily serves clients within the five boroughs of New York City (Manhattan, Brooklyn, Queens, The Bronx, and Staten Island).
However, if you're located outside of the five boroughs, please don't close this form.
Some matters require New York licensure, in-person court filings, and local knowledge; others, like contract review, can be handled regardless of where you're located. If I'm not the right fit, I'll do my best to connect you with a trusted referral in your area; you won't leave our conversation without a next step.

BEFORE WE CONTINUE
Are you an older youth looking for free, limited legal guidance?
This question is for young people who may be dealing with big life changes without a lot of steady support.
You may be in the right place if you are an older youth, including a youth in shelter or transitional living, an independent minor, a young person aging out of care, or someone getting ready to turn 18 and trying to understand what legal paperwork or public benefits may matter for you.
This may include help understanding:
public benefits forms, like SNAP/EBT, Medicaid, Cash Assistance, or similar programs;
legal paperwork connected to turning 18, like health care proxy, HIPAA authorization, FERPA guidance, or power of attorney questions;
basic civil legal questions;
letters, forms, or notices you do not understand;
what questions to ask before signing or submitting something;
whether another legal services provider or community resource may be a better fit.
You do not need to know legal words to ask for help. You do not need to have everything figured out. This screening is here to help us understand what is going on and whether this is something we may be able to help with.
Yes, I’m seeking help for myself
Yes, I’m helping an older youth seek support
I’m not sure
No, I’m seeking regular Lewis Legacy Law Studio services
If you answered “Yes” or “I’m not sure,”
please pause here.
You may be eligible for a separate youth legal guidance screening connected to Nisey’s Nurture Network. That screening form is shorter, easier to follow, and made specifically for older youth seeking free, limited-scope legal guidance.
Click here to complete the Nisey’s Nurture Network Youth Legal Guidance Screening Form.
Free legal guidance is limited and capacity-based. It is provided separately through Lewis Legacy Law Studio only after eligibility review, conflict check, and a written limited-scope agreement. Completing this form or the screening form does not create an attorney-client relationship.
WHAT BRINGS YOU HERE?
What area do you need help with? (Check all that apply)
Estate planning: wills, advanced directives, health care proxy, power of attorney
Business formation: LLC setup, operating agreements, startup contracts
Contracts: review, drafting, or explanation of an existing agreement
Nonprofit governance: bylaws, board compliance, governance documents
Special education: help understanding your child's rights, IEP review, and what to ask for at the table (advisory and counseling, not litigation)
Turning 18 legal readiness: understanding what legally changes at 18, key documents for young adults (health care proxy, financial power of attorney, HIPAA authorization, FERPA guidance), and plain-language advisory on guardianship vs. supported decision-making (advisory only)
Family matters: visitation, mediation support, guardianship documentation, and family legal guidance (advisory and counseling, not litigation)
Housing: help understanding your lease, your rights as a tenant, and rent stabilization protections (advisory and counseling, not litigation)
Public benefits: navigating benefit programs or hearings
Not sure yet, I just know I need legal help
Other (please describe below)
In a few sentences, tell me what’s going on:
Plain language is perfect. You don’t need legal terms, just tell me what’s happening and what you’re hoping to figure out.
How urgent is your situation?
This helps me prioritize scheduling. If something is time-sensitive, please note that below
It’s time-sensitive: I have a deadline or immediate concern
It’s important, but not an emergency
I’m planning ahead; there's no immediate urgency
Relevant deadline or date (if it applies)
HOW DID YOU HEAR ABOUT US?
- Personal referral: a friend, family member, or colleague?
- Professional referral: another attorney, organization, or service provider?
- Church or community organization?
- Social media (Instagram, Facebook, LinkedIn, etc.)?
- Web search?
- Workshop or community event?
Not required, but it's always appreciated; so if referred by someone, I'd love to know whom I should thank!
HOW YOU WORK BEST
Everyone works differently, and I want our process to fit you - not the other way around. If we move forward after our discovery call, I’ll send you a more detailed intake form specific to your legal matter. The question below helps me understand how to best support you with that.
When it comes to paperwork, which feels more comfortable?
There’s no right answer here. Both options are fully supported; this just helps me plan our time together.
I’m comfortable completing forms on my own and returning them before our appointment
I’d prefer to go through the form together on our call - it helps to have someone walk me through it
Either works for me
A NOTE ON ACCESS
Lewis Legacy Law Studio reserves a limited number of adjusted-fee engagements each month for clients who are income-eligible and cannot meet standard rates.
If cost is a concern, please note that below and we will discuss your options during the discovery call.
This program is distinct from pro bono services and is available on a first-come, first-served basis
Yes, I'd like to discuss my options
ANYTHING ELSE
Is there anything else you’d like me to know before our call?
A concern, a question, something you want to make sure we cover - this is the place to tell me.
To acknowledge that you've read the privacy policy above regarding use of your personal information, type your full legal name below
